Office Policies
Welcome to our clinic! We are excited to provide you with our services and we hope that you can feel at home at our practice. If you have had x-rays within the last six months fill out the release of record below and send it back to us so we can contact your previous dentist for your x-rays. Please call us for our fax or email address. Additional films may be requested to better serve our patients if we do not have current x-rays.
Please remember as a courtesy to other patients and staff we require that you call 48 hours in advance (2 business days) if you are unable to attend your reserved appointment. Please note that after two failed or cancelled appointments within less than 2 business days, your account will be charged $50.
It is our goal to make an individualized treatment plan based on your dental needs. We are happy to submit your dental insurance claims and request estimates on large treatment plans on your behalf. If you have any specific questions, please feel free to call your insurance company directly. Co-payments are due at the time of treatment for the estimated portion that will not be covered under your insurance. Please note that we do not have access to your balances from other specialists or dental clinics, so you may owe more once we get reimbursed from your insurance company especially if you used your yearly maximum.
We accept three payment options. 1. Online (Look on our payments page) 2. Mailing in a payment, or 3. Deliver it to the office.
Our office accepts cash, personal check with ID, and credit cards (Discover, MasterCard, and Visa).
All return checks will have a $20 fee. Any balances that have gone unpaid after 60 days will be subject to a 1.5% month/18% annually.
What to bring to your first appointment:
1. Driver's license
2. Insurance card (if applicable)
3. List of Medications
4. Make sure to have your Release of Records filled out if you have had x-rays within last 6 months.
If you need a Pre-Medication please let us know.
